MENTAL HEALTH COUNSELING INFORMED CONSENT AND CLINICAL ENGAGEMENT AGREEMENT This Informed Consent Agreement (the "Agreement") is executed on this [Informed Consent Date], by and between: MENTAL HEALTH PROFESSIONAL: [Counselor / Psychologist Full Name], possessing professional qualifications: [Professional Qualifications & Accreditations], registered under [RCI / Professional Council Registration Number] (hereinafter referred to as the "Counselor", which expression shall include legal representatives) of the FIRST PART; AND CLIENT: [Client Full Name], aged [Client Age] years (hereinafter referred to as the "Client", which expression shall include legal heirs and representatives) of the SECOND PART. WHEREAS: A. The Counselor is a licensed mental health practitioner providing evidence-based psychological counseling and psychotherapy; B. The Client desires to participate in mental health counseling and undertakes therapy with informed understanding of the rights, responsibilities, and clinical limits described herein. NOW, THEREFORE, IN COMPLIANCE WITH THE MENTAL HEALTHCARE ACT 2017 AND THE INDIAN CONTRACT ACT 1872, THE PARTIES AGREE AS FOLLOWS: 1. NATURE AND GOALS OF COUNSELING 1.1 Counseling and psychotherapy are collaborative clinical processes aimed at addressing psychological distress, cognitive patterns, behavioral changes, interpersonal issues, and emotional well-being. 1.2 The Client understands that therapy involves discussing sensitive personal matters, which may occasionally trigger temporary emotional distress or discomfort before beneficial insights are integrated. 1.3 Mode of Delivery: Sessions shall be held via In-Person Clinic Sessions at a scheduled cadence of [Session Frequency & Typical Duration]. 2. STATUTORY CONFIDENTIALITY AND MANDATORY EXCEPTIONS 2.1 Confidentiality Standard: Full Confidentiality under Mental Healthcare Act 2017. 2.2 In accordance with Section 23 of the Mental Healthcare Act 2017, all communications, clinical notes, and records generated during therapy are strictly confidential and will not be disclosed to any third party, employer, family member, or institution without the prior written consent of the Client. 2.3 Statutory Mandatory Exceptions: The Client understands that confidentiality is not absolute and must be broken by the Counselor under the following legal exceptions: a) Imminent Risk of Serious Harm to Self: Where the Counselor has reason to believe the Client poses an imminent threat of suicide or grievous self-harm; b) Imminent Risk of Serious Harm to Others: Where there is an explicit threat to the physical safety of an identifiable third person; c) Child or Vulnerable Adult Abuse: Mandatory reporting obligations under the Protection of Children from Sexual Offences (POCSO) Act 2012 or related statutes; d) Lawful Court Orders: When compelled by a valid subpoena or judicial directive from a court of competent jurisdiction. In such events, only the minimal information necessary to ensure safety will be shared with the designated emergency contact ([Designated Emergency Contact Person & Phone Number]) or statutory authorities. 3. CRISIS AND NON-EMERGENCY PROTOCOL 3.1 The Counselor is not a 24-hour psychiatric emergency helpline. 3.2 In case of acute emotional distress, active suicidal ideation, or psychiatric emergency outside scheduled sessions, the Client must contact: - Designated Emergency Contact: [Designated Emergency Contact Person & Phone Number] - National Mental Health Helpline (Tele-MANAS): 14416 or 1800-891-4416 (24x7 toll-free) - National Emergency Services: 112 - Or report immediately to the nearest hospital psychiatric casualty department. 4. PROFESSIONAL FEES AND CANCELLATION 4.1 The Client shall pay a fee of INR [Fee per Consultation Session (in INR)]/- (Indian Rupees [Fee per Consultation Session (in INR)] only) per scheduled 50-minute session. 4.2 Fees are payable on or before each session via UPI, Net Banking, or Card. 4.3 Cancellations or rescheduling require a minimum of 24 hours advance notice. Cancellations made with less than 24 hours notice or missed appointments without notification will be billed at the full session rate. 5. THERAPEUTIC BOUNDARIES AND CODE OF CONDUCT 5.1 The relationship between Counselor and Client is strictly professional. Dual relationships, social friendships, business partnerships, or personal social media interactions are ethically impermissible. 5.2 If sessions take place online, the Client must ensure they are in a private, quiet room with a stable internet connection to safeguard confidentiality. 6. VOLUNTARY PARTICIPATION AND TERMINATION 6.1 Therapy is entirely voluntary. The Client retains the right to terminate counseling at any time. 6.2 When terminating, it is recommended to conduct at least one closing session to review progress and ensure safe psychological closure. 7. GOVERNING LAW 7.1 This Agreement is governed by the laws of India, including the Mental Healthcare Act 2017 and the Indian Contract Act 1872. 7.2 Any disputes shall be subject to the exclusive jurisdiction of the competent civil courts at the Counselor's city of practice. SCHEDULE OF TERMS, COVENANTS & CONDITIONS(6 ACTIVE CLAUSES)

1. TENURE & TERM OF LEASE: The term of this lease/tenancy shall be for a fixed duration of {{duration_months}} months commencing on {{commencement_date}} and terminating on {{expiry_date}} unless renewed by mutual consent.

6. SUB-LEASE & ASSIGNMENT PROHIBITION: The Second Party is strictly prohibited from sub-letting, parting with possession, or assigning any portion of the premises to third parties without prior written consent.
