ELDER CARE ATTENDANT SERVICE AGREEMENT This Elder Care Attendant Agreement (the "Agreement") is made on this [Agreement Date], by and between: EMPLOYER / SPONSOR: [Employer / Family Guardian Full Name] (hereinafter referred to as the "Employer", acting on behalf of the senior citizen ward) of the FIRST PART; AND CARE ATTENDANT: [Elder Care Attendant Full Name], holding identification document [Attendant ID Proof (Aadhaar / Voter ID)], representing [Care Agency / Independent Practice] (hereinafter referred to as the "Attendant", which expression shall include legal heirs and representatives) of the SECOND PART. WHEREAS: A. The Employer desires to retain compassionate, trained attendant care for senior citizen [Senior Citizen / Ward Full Name], aged [Senior Citizen Age] years; B. The Attendant agrees to deliver daily assisted living support and companionship in accordance with the terms below. NOW, THEREFORE, THE PARTIES MUTUALLY AGREE AS FOLLOWS: 1. APPOINTMENT AND SCOPE OF ELDER ASSISTANCE 1.1 The Employer engages the Attendant to provide personal caregiving assistance to [Senior Citizen / Ward Full Name]. 1.2 Scope of Care: The Attendant shall assist with: "[Assistance and Care Requirements]" 1.3 Non-Medical Scope: The Attendant acknowledges that their role is non-clinical caregiving. The Attendant shall remind the senior citizen to take prescribed medications at scheduled times, but shall not independently alter dosages, inject medications, or perform invasive clinical procedures. 1.4 Standard of Conduct: The Attendant shall treat the senior citizen with utmost dignity, patience, empathy, and respect. Any form of verbal disrespect, financial manipulation, emotional neglect, or physical roughness is strictly forbidden. 2. DUTY TIMINGS AND LEAVE 2.1 Duty Schedule: Services shall be rendered during [Duty Hours and Schedule]. 2.2 Leave Policy: The leave arrangement shall follow: "[Leave and Absence Policy]" 2.3 In the event of illness or scheduled leave, the Attendant or Agency shall notify the Employer in advance so that suitable relief arrangements can be coordinated. 3. COMPENSATION AND WAGES 3.1 The Employer agrees to pay the Attendant / Agency monthly remuneration of INR [Monthly Wages / Charges (in INR)]/- (Indian Rupees [Monthly Wages / Charges (in INR)] only). 3.2 Remuneration shall be paid on or before the 5th day of every calendar month via Bank Transfer or UPI. 4. MEDICAL EMERGENCY PROTOCOL 4.1 In the event of a medical emergency, fall, sudden loss of consciousness, chest pain, or respiratory distress, the Attendant shall act promptly according to: "[Medical Emergency Protocol]" 4.2 The Attendant shall remain by the side of the senior citizen until medical assistance arrives or family members assume direct supervision. 5. CONFIDENTIALITY AND FINANCIAL SEPARATION 5.1 Privacy Commitment: Strict Patient Privacy and Non-Disclosure. 5.2 The Attendant shall not handle the senior citizen's chequebooks, bank passbooks, ATM cards, debit cards, cash lockers, or gold jewelry. 5.3 The Attendant shall keep all family financial affairs, legal matters, and domestic discussions strictly confidential. 6. BACKGROUND VERIFICATION 6.1 The Attendant confirms that they possess valid government photo identification ([Attendant ID Proof (Aadhaar / Voter ID)]) and have no past criminal convictions or pending police complaints. 7. TERMINATION AND NOTICE 7.1 Notice Period: [Termination Notice]. 7.2 The Employer reserves the right to terminate this Agreement immediately with forfeiture of notice period in the event of elder abuse, theft, unauthorized absence, or intoxication. 8. GOVERNING LAW 8.1 This Agreement is governed by the laws of India, including the Indian Contract Act 1872 and the principles under the Maintenance and Welfare of Parents and Senior Citizens Act 2007. 8.2 Any dispute shall be submitted to the exclusive jurisdiction of the competent civil courts at the Employer's city of residence. SCHEDULE OF TERMS, COVENANTS & CONDITIONS(6 ACTIVE CLAUSES)

1. TENURE & TERM OF LEASE: The term of this lease/tenancy shall be for a fixed duration of {{duration_months}} months commencing on {{commencement_date}} and terminating on {{expiry_date}} unless renewed by mutual consent.

6. SUB-LEASE & ASSIGNMENT PROHIBITION: The Second Party is strictly prohibited from sub-letting, parting with possession, or assigning any portion of the premises to third parties without prior written consent.
